Planning for your office visit
Please remember to bring your health insurance card, photo ID, all medication bottles or list of current medications. Co-pay is due at the time of the visit. If you are a new patient, please bring the contact information of all the medical offices from where we may have to request medical records. You may fill out the necessary forms ahead of time! Click on the “Coming early on your first visit allows time to fill out any other the necessary forms.
We understand that the unexpected happen. If you need to cancel your appointment, please call us at least 24 hours before your appointment time. Failure to notify our office will result in a fine. Skipping appointments without notifying us causes difficulty in scheduling, failure of other patients to be able to receive earlier appointments and cost all of our staff energy, time, and resources that could be used to help others. Please be kind and notify us as soon as you know you will not make an appointment.
Please arrive on time for your appointments, especially if you are a new patient. If you are late, we will still try to accommodate you but this may not always be possible. We also try our best to be available for same day appointments. Please call us to avoid the unnecessary hassle of waiting in the Emergency Room for non-emergency conditions.
For all prescription refills, you or your pharmacist can call during working hours when your records are available. Please call for refills about one week before you run out of medication. In general, you must be seen in the office regularly if you are taking prescribed medications.
We return calls as soon as possible. We are easily accessible over weekends and other holidays for urgent problems. However, we do request patients to call during office hours for non-urgent matters. If your problem is urgent, we will arrange for you to be seen at the Clinic the same day or referred to a local hospital if needed. We will work closely with the Emergency room physicians to ensure that your needs are met. If hospitalized you will be seen by us promptly.
